How to Join:
If you’re interested in becoming a member of Crossroads, we strongly encourage you to come in for a tour. You may call to schedule a tour, (918)-749-2141, or stop by any time Monday-Friday between the hours of 9:00 a.m.-5:00 p.m.
Membership is available to those who meet the following basic criteria:
- Must be at least 18 years of age
- Must have an Axis I serious mental illness diagnosis
If you’re interested in becoming a Crossroads member, there are two ways to complete an application.
1. Print off a hard copy of our application by clicking HERE. Pages 9-10 of the application MUST be filled out by a credentialed treatment provider. Completed applications can be mailed, faxed (918-749-2150), or dropped off at the Clubhouse during regular business hours.
2. Fill out an application online WITH your treatment provider by clicking on the link below.
